New client referrals must go through the Central Referral Hub. As Nurse Practitioner Ann-Marie attends thorough diabetes assessments, education and management and follow up for patients after discharge from Peninsula Private Hospital. She demonstrates expertise in all areas of diabetes management including Type 1, Type 2, Gestational Diabetes, Diabetes in pregnancy, insulin pump therapy, and continuous glucose monitoring. Fee for service provided. No care plans are required due to Nurse Practitioner endorsement, all that is required for a referral is a thorough referral letter and a Medicare rebate applies.
