EdHealth Australia is a NDIS Registered Provider. We focus on the disability side of a person’s diabetes care to set each person we work with, on a path to successfully manage their diabetes. We match each person and their Circle of Support with services that are individualised and person centred, for diabetes success. The work we do is unique and effective. We enable people with a disability and their team to access the same standard of contemporary diabetes management, support and education as people without a disability. This service is available to NDIS Participants across Australia.

I no longer provide Chronic Disease Management (CDM) services. If you are a NDIS Participant, you may be able to use your CDM referrals for services to complement those provided by your NDIS funding.
